Teresa A. Fowlkes

Teresa Ann Lewis Fowlkes, age 58, was born on October 16, 1967, to Oliver Vincent Lewis and Becky Diane Lewis. She peacefully departed this life on December 16, 2025, just two months after her birthday, leaving behind a legacy filled with love, care, and dedication to others. Teresa began her education at School 83, continued at Creston Middle School, and later attended Warren Central High School, where she graduated in 1985. During her high school years, she truly came into her own. She was confident, full of life, and well loved by those around her. Teresa was very popular and part of a close group of friends known as the “Bad Girls Crew,” friendships she cherished deeply. After high school, Teresa continued her education at Ivy Tech, where she graduated in the late 1990s as a Licensed Practical Nurse (LPN). She dedicated her career to caring for others and worked as an LPN at The Forum at the Crossing Nursing Facility for over 21 years until it closed. Teresa was known for her patience, compassion, and strong work ethic, and she touched many lives through her work in healthcare. Faith and family were at the center of Teresa’s life. She attended Antioch Baptist Church with her parents and extended family, where she was a member of the choir in her younger years alongside her brother, cousins, and mother. Teresa was also known for opening her home to others. She loved hosting annual barbecues and neighborhood gatherings, creating a warm and welcoming space filled with laughter, love, and community. Teresa shared a deep and lasting love with her husband, Andrew Fowlkes Jr. Where the relationship began in 1995 and grew stronger over the years through friendship, commitment, and shared experiences. After nearly twenty years together, they were united in marriage on August 1, 2014, in a private and intimate ceremony that reflected the strength of their bond. Teresa was a devoted mother to Tiara Banks and Jerry Banks Jr., a loving stepmother to Andrew Fowlkes III, and a proud grandmother to ten grandchildren who brought her endless joy. She shared especially close relationships with her family, including her Aunt Cheri, who was like a second mother to her, as well as Aunt Inez and Aunt Donna. Teresa also had a sister-like bond with her cousins Heaven (“Tater”) and Jacqueline (“Cissy”), having grown up together more like siblings than cousins. She was also a loving godmother and a cherished aunt, cousin, and friend. Teresa was preceded in death by her parents, Oliver Vincent Lewis (2019) and Becky Diane Lewis (1986); her sister, Cynthia Powell; all of her grandparents; and other beloved family members. She leaves to cherish her memory with loving husband, Andrew Fowlkes Jr.; her children, Tiara Banks and Jerry Banks Jr.; her stepson, Andrew Fowlkes III; her brother, Oliver Lewis Jr.; her grandchildren; and a host of nieces, nephews, cousins, extended family, and friends. Teresa Ann Lewis Fowlkes will always be remembered for her beautiful smile, generous heart, and the way she made everyone feel welcome. Her love and spirit will live on in all who knew her.
